This role involves providing a high quality and structured reablement service for clients discharged from hospital, considering their rights, privacy, dignity, and independence. To ensure they are fully involved in their independence goals and care delivery encouraging them to do as much as possible for themselves. Working alongside of other professionals. A rewarding and valuable role in your local community.

Shift Patterns available, 7am - 3pm, 3pm - 10pm

This position offers full time salaried contract -   £21,064.00 per year 

Please note, due to the nature of the position access to a vehicle is essential. 

Ability to reliably commute or plan to relocate to Newport before starting work is required.

At least one-year domiciliary care experience is required for this position.
